Job summary

Adaptive Home Health seeks an experienced RN Case Manager to deliver skilled home health care across Texas. You will assess patients, manage wound care and infusion therapy, and coordinate care with physicians, families and the care team.

Youll drive patient outcomes with autonomy, supported by a clinical team and our charting tools. The position offers field-based work, mileage in addition to productivity, and a competitive pay model.

Qualifications

• Current Texas RN license.
• 3+ years clinical nursing experience.
• Comfort with wound care and infusions.
• CPR required (BLS).
• Current Texas driver’s license and field-based work.
• Strong time management and organization.
• Clear, professional communication with patients, families, and providers.

Responsibilities

• Own patient assessments and skilled visit documentation including OASIS.
• Deliver high-acuity home health care, with focus on wound care and infusion therapy.
• Build and maintain a clear plan of care through communication with physicians and families.
• Advocate for patients and ensure continuity of care across disciplines.
• Drive case management with care coordination and timely follow-ups.
• Coordinate services (PT/OT/ST/MSW/HHA) to execute the plan of care.
• Administer medications and provide patient and caregiver teaching.
• Supervise LVNs and HHAs when applicable.
• Participate in case conferences and in-services to align on outcomes.
