Job summary

The ideal candidate will hold an unrestricted Colorado RN license and have a minimum of three years' clinical experience, with strong skills in wound care and infusion therapy. Flexible scheduling and clinical support are integral to this position.

Qualifications

• Must have a current Colorado RN license without restrictions.
• Minimum of 3 years of clinical nursing experience is required.
• Comfort with wound care and infusion treatments.
• BLS certification is necessary.
• Must possess a current driver's license and be comfortable with field-based work.
• Exemplary time management and organizational skills in a high-autonomy work environment.

Responsibilities

• Conduct patient assessments and document skilled visits including OASIS.
• Deliver high-quality home health care centered on wound care and infusion therapy.
• Coordinate treatment plans and communicate with healthcare professionals and patients.
• Advocate for patients to ensure continuity of care.
• Accomplish case management duties including timely follow-ups.
