Job summary

Adaptive Home Health is seeking a Registered Nurse (RN) - Case Manager based in Littleton, Colorado. This full-time position involves delivering skilled home health care with a focus on wound care and infusion therapy while advocating for patients. You'll work autonomously, supported by a strong clinical team to ensure quality care.

The ideal candidate will have a current Colorado RN license, at least 3 years of nursing experience, and the ability to manage time and communicate effectively with patients and families.

Qualifications

• Must have a current Colorado RN license and 3+ years of clinical nursing experience.
• Comfort with wound care and infusions is required.
• CPR certification (BLS) and a driver's license needed.

Responsibilities

• Own patient assessments and documentation including OASIS.
• Deliver high-acuity home health care focusing on wound care and infusion therapy.
• Coordinate care and communicate changes in patients' conditions.
