Qualifications

• 3+ years of clinical nursing experience.

• Comfort working with multiple systems and documentation platforms.

Responsibilities

• Coordinate care across providers to ensure a seamless member experience.
• Assess health and social needs via telephonic and face-to-face visits.
• Develop personalized care plans for short- and long-term wellness.
• Identify risks and gaps using data insights to inform intervention.
• Support members with complex conditions using a holistic approach.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences with multidisciplinary teams.
• Review claims history and eligibility to inform care planning.
• Engage members to promote active participation in their care.
