Job summary

You will assess needs, develop care plans, coordinate across providers, review claims, and support members with complex conditions. Travel up to 75% with mileage reimbursed; 40-hour week, 8-5 EST schedule.

Qualifications

• Active, unrestricted RN license in Virginia.
• Residence in Northern Virginia.
• Willingness to travel up to 75% with mileage reimbursed.
• 3+ years of clinical nursing experience.

Responsibilities

• Conduct comprehensive assessments (telephonic and in-person) to evaluate medical, behavioral, and social needs.
• Develop and implement personalized care plans to support short- and long-term wellness.
• Coordinate care across providers and services for a seamless member experience.
• Identify risks and gaps in care using clinical data and insights.
• Support members with complex conditions using a holistic approach.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences with multidisciplinary teams.
• Review claims history and eligibility to inform care planning.
• Engage members to encourage active participation in their care.
• Ensure case management activities comply with regulations.
