Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager to support Northern Virginia. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care. Schedule: Monday-Friday, 8:00 AM - 5:00 PM EST.

The typical pay range is $60,522.00 - $129,615.00 with CVS Health bonus eligibility. Active Virginia RN license required, 3+ years of clinical nursing experience, and ability to travel up to 75% of the time.

Qualifications

• Active, unrestricted RN license in Virginia.
• Residence in Northern Virginia.
• Willing to travel up to 75% of the time with mileage reimbursed.
• 3+ years of clinical nursing experience (medical-surgical, acute, or behavioral).
• Strong communication, organization, and critical-thinking skills.
• Comfort with using multiple systems and documentation platforms.

Responsibilities

• Conduct comprehensive telephonic and face-to-face assessments of members’ medical, behavioral, and social needs.
• Develop and implement personalized care plans to promote short- and long-term wellness.
• Coordinate care across providers and services for a seamless member experience.
• Use clinical expertise and data insights to identify risks and intervention opportunities.
• Support members with complex conditions using a whole-person approach.
• Facilitate referrals to clinical and community resources.
• Participate in case conferences and collaborate with multidisciplinary teams.
• Review claims history and benefit eligibility to inform care planning.
• Engage members to build trust and encourage active participation in care.
• Ensure all case management activities comply with regulatory guidelines.
