Job summary

CVS Health is seeking a compassionate Registered Nurse Case Manager to support Northern Virginia. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care. Schedule is Monday–Friday, 8:00 AM–5:00 PM EST, with 40 hours weekly.

The role requires an active VA RN license, 3+ years of nursing experience, and willingness to travel up to 75%. BSN preferred.

Qualifications

Active, unrestricted RN license in Virginia.
Residence in Northern Virginia.
Willingness and ability to travel up to 75% with mileage reimbursement.
3+ years of clinical nursing experience (medical-surgical, acute care, or behavioral health).
Strong communication, organization, and critical-thinking skills.
Comfort working with technology and multiple documentation platforms.

Responsibilities

Conduct comprehensive assessments (telephonic and face-to-face) to evaluate members' medical, behavioral, and social needs.
Develop and implement personalized care plans for short- and long-term wellness.
Coordinate care across providers, programs, and services for a seamless member experience.
Identify risks and gaps in care using clinical expertise and data insights.
Support members with complex conditions through a holistic, whole-person approach.
Facilitate referrals to clinical and community resources.
Participate in case conferences and collaborate with multi-disciplinary teams.
Review claims history and benefit eligibility to inform care planning.
Engage members to build trust and promote active participation in care.
Ensure all case management activities comply with regulatory and organizational guidelines.
