Job summary

Castor Home Health Solutions seeks a Registered Nurse (RN) Care Manager for a full-time, field-based role serving Central Illinois, including Peoria and surrounding counties. You will coordinate patient care, provide clinical oversight, mentor field nurses, and ensure quality patient outcomes.

This role includes a company vehicle and travel within the region, with strong collaboration with clinical leadership and a supportive team environment.

Qualifications

• Active Illinois RN license required.
• Pediatric, Private Duty Nursing, Home Nursing, Home Health, or care management experience preferred, but not required.
• Strong communication, organization, and relationship-building skills.
• Ability to work independently and manage multiple priorities.
• Comfortable with a field-based position and regional travel.
• Valid driver’s license and acceptable driving record required.

Responsibilities

• Provide clinical oversight and care coordination for assigned patients.
• Complete assessments, admissions, supervisory visits, and other clinical visits as needed.
• Support and maintain strong relationships with patients, families, and field nurses.
• Assist with nurse orientation, education, training, and competencies.
• Monitor clinical documentation and plans of care.
• Collaborate with clinical leadership and other members of the care team.
• Support quality, compliance, and continuity of care.
• Perform other clinical and administrative responsibilities as assigned.
