Job summary

UnitedHealth Group's UnitedHealthcare is seeking a Registered Nurse Care Coordinator to join its integrated care team in Hawaii. In this role you will assess patient needs, plan care strategies, coordinate long‑term care services, and advocate for patients to ensure access to appropriate resources.

If located on Oahu, HI, you may work remote and travel locally up to 75% across Honolulu and surrounding areas.

Qualifications

• Current, unrestricted RN license in Hawaii
• 2+ years of clinical experience
• Experience with long‑term care, home health, hospice, public health or assisted living
• MS Word, Excel and Outlook proficiency

Responsibilities

• Assess, plan and implement care strategies individualized by patient and directed toward appropriate care level
• Identify and initiate referrals for social service programs
• Manage the care plan throughout the continuum of care as a single point of contact
• Communicate health-related information to stakeholders to ensure coordinated care
• Advocate for patients and families to ensure needs are represented and supported by the care team
