Job summary

aFit is seeking a Regional Grant Coordinator to support Indiana’s Rural Health Transformation Program. You will manage grants, contracts, and regional partnerships to turn statewide strategy into local action, ensuring compliance and timely reporting.

You will serve as the liaison among state agencies, regional coalitions, hospitals, clinics, EMS, and community organizations, guiding planning, execution, and data-driven improvements in access to care across rural Indiana.

Qualifications

• Bachelor’s degree in public health, health administration, public policy, business administration, or related field – Required
• Experience in grant management, contract oversight, program administration, or related work – Required, 5 years
• Experience working with healthcare organizations, rural communities, or public health systems – Required, 3 years
• Strong understanding of compliance, budgeting, and reporting requirements for state or federal grants – Required, 2 years
• Ability to manage multiple projects simultaneously with strong attention to detail – Required
• Excellent communication, facilitation, and relationship-building skills – Required
• Ability to work independently in a remote, field-based environment – Required
• Must reside within the assigned RHTP region and be willing to travel regularly within the region – Required
• Experience with CMS-funded programs or federal cooperative agreements – Highly Desired
• Familiarity with Indiana’s rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations – Highly Desired
• Experience collaborating with statewide associations or multi-stakeholder coalitions – Highly Desired
• Knowledge of rural health delivery, care coordination, or health system transformation – Highly Desired

Responsibilities

• Grant & Contract Management: Serve as the primary regional contract manager for organizations receiving RHTP funding
• Partner Coordination: Collaborate with statewide Technical Assistance Providers (IHA, IPHCA, IRHA)
• Reporting & Performance Tracking: Coordinate data collection and performance tracking aligned with CMS requirements
• Stakeholder Engagement: Serve as the day-to-day liaison between IDOH and regional partners
• Program Implementation: Help partners interpret program guidance and funding requirements
