• Assist consumers with identifying services, accessing community resources, managing budgets, and making appropriate purchasing decisions.
• Support consumers with hiring, training, supervising, and terminating support staff as needed.
• Collaborate with healthcare providers, community-based organizations, service providers, consumers, and representatives.
• Maintain accurate case documentation, records, appointments, deadlines, and field visit schedules.
