Job summary

Adaptive Home Health, LLC is seeking a Physical Therapist to deliver skilled home health PT in the Denver area. The role is field-based with a full-time schedule Monday–Friday plus occasional weekends.

You will own the caseload, perform in-home assessments, and drive outcomes with a focus on gait, balance, and safe home mobility. Requirements include an active Colorado PT license and strong independent field-work capabilities.

Qualifications

• Active Colorado PT license in good standing.
• Strong clinical judgment and comfort working independently in the field.
• Comfortable escalating changes in condition to RN/MD and documenting rationale clearly.

Responsibilities

• Own the caseload as a case manager: therapy plan, progression, outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety.
• Set measurable goals and track progress; adjust treatment when needed.
• Drive multidisciplinary coordination with care team and orders.
• Manage treatment and outcomes, including PTA supervision where appropriate.
• Coach patients/caregivers with practical home programs.
• Document promptly in Homecare Homebase.
