Job summary

Adaptive Home Health in Colorado is seeking a Physical Therapist (PT) for a full-time position. As a PT, you will own the caseload as a case manager and be accountable for therapy plans, delivering in-home assessments, and driving patient outcomes.

Qualifications

• Active Colorado PT license in good standing.
• Strong clinical judgment and comfort working independently.
• Comfortable escalating changes in condition and documenting rationale clearly.
• Strong communication and documentation skills.
• Reliable follow-through on orders, frequencies, and patient goals.
• Current driver's license and comfort with daily travel across assigned territory.
• Able to carry/transport basic therapy equipment as needed.
• Competency in core home health PT scope.
• Safety-first mobility work: transfers, gait training, balance/fall prevention.
• Strengthening and home safety assessment/modification education.
• Patient/caregiver coaching with practical home programs.

Responsibilities

• Own the caseload as a case manager, accountable for the patient’s therapy plan and outcomes.
• Complete comprehensive in-home assessments and deliver skilled PT visits.
• Build and execute a clear plan of care: gait/balance, transfers, strengthening.
• Set measurable goals, track objective progress, and adjust treatment when necessary.
• Drive multidisciplinary coordination and communicate changes in condition.
• Manage treatment and outcomes directly, including PTA supervision.
• Coach patients and caregivers with practical home programs.
• Document promptly and thoroughly in Homecare Homebase.
