Job summary

Coastal Horizons Center, Inc. seeks a Peer Support Specialist - Case Manager to join our Post Overdose Response Team, helping overdose survivors access treatment and safe housing where available.

You will work with EMTs, community partners, and clinical staff to provide recovery-based support, coordinate care, and empower clients through education and trusted relationships.

This full-time role requires willingness to transport clients and conduct home visits as part of comprehensive outreach.

Qualifications

• Personal history of lived addiction/recovery and consumer of behavioral health services or knowledge of care and wellness plan coordination.
• Two (2) years of recovery with mental health or substance use required.
• PSS Certification/WRAP Preferred.

Responsibilities

• Coordinate with EMS, Harm Reduction, Sober Living facilities and community partners to advance the PORT program.
• Provide recovery-based support to assist overdose survivors in accessing treatment or harm reduction services.
• Link clients to medication management, coordinate care, educate clients and model positive behavior.
• Support continuity of care and connect clients to community recovery programs and resources.
• Teach, mentor and monitor clients on issues relevant to their care.
