Job summary

Coastal Horizons seeks a Peer Support Specialist - Case Manager to join the PORT team in NC. The role focuses on linking overdose survivors to treatment, harm reduction services, and community resources.

Responsibilities include outreach, care coordination, education, and mentoring clients to help achieve stability. Requires lived recovery experience or related knowledge, with two years of recovery preferred.

Qualifications

• Personal history of lived addiction/recovery or demonstrated knowledge of care and wellness plan coordination.
• Two (2) years of recovery with mental health or substance use required.
• PSS Certification/WRAP preferred.

Responsibilities

• Provide recovery-based support for PORT to assist overdose survivors in accessing treatment or harm reduction services.
• Develop connections with EMTs, Harm Reduction, Sober Living facilities and community partners.
• Coordinate care, educate clients, and connect them to resources and recovery programs.
• Monitor and mentor clients, build trust with clinical teams, and support continuity of care.
