Job summary

Coastal Horizons Center, Inc. in Kentucky seeks a Peer Support Specialist - Case Manager to join our PORT integrated care team, helping overdose survivors access treatment and harm reduction services.

The role uses lived experience to build trust, connect clients to medication management and community resources, and coordinate care with clinical staff and partners while teaching and mentoring clients.

Qualifications

• Personal history of lived addiction/recovery and as consumer of behavioral health services preferred, or experience/demonstrated knowledge of care and wellness plan coordination, in addition to local resources.
• Two (2) years of recovery with mental health or substance use required.
• PSS Certification/WRAP Preferred.

Responsibilities

• Connect with EMS, shelters, and community partners to develop referrals.
• Provide recovery-based support to help access treatment and harm reduction.
• Link clients to medication management and coordinate care.
• Educate clients and mentor them using lived experience.

Education

Two (2) years of recovery with mental health or substance use
