Job summary

Adaptive Home Health in Colorado is seeking an Occupational Therapist (OT) for a field-based, full-time role, Monday–Friday with occasional weekends. You will perform in-home evaluations and deliver skilled OT visits to restore function in the home.

You will develop plans of care focused on ADLs/IADLs, safety and independence, recommend adaptive equipment, and coordinate with nursing and the care team. Strong clinical judgment and independent field work are essential; you’ll document thoroughly

Qualifications

Active Colorado OT license in good standing.
Strong clinical judgment; able to work independently in the field.
Comfort escalating changes to RN/MD and documenting rationale clearly.
Strong communication and documentation skills.
Reliable follow-through on orders, frequencies, and patient goals.
Driver’s license and daily travel across assigned territory.
Able to carry/transport basic therapy equipment.
Competency in core home health OT scope (ADL/IADL, safety, equipment, caregiver education).

Responsibilities

Complete in-home OT evaluations and deliver skilled OT visits.
Build and execute a plan of care focused on ADLs/IADLs, safety, and independence.
Recommend and train on adaptive equipment and home modifications.
Coordinate with nursing and care team when needs change.
Document promptly and thoroughly.
