Castle Rock Home Care Inc. is seeking an experienced OPWDD Community Marketing Representative to expand our pediatric and developmental disability services across New York City.

This field-based role focuses on building relationships with care managers, schools, hospitals, and community organizations to drive referrals and SOC growth. You will educate referral sources on available home care, nursing, therapy, and support services, while collaborating with intake and clinical teams to ensure

Qualifications

• Active OPWDD care management experience required.
• Experience in home care, healthcare marketing, care coordination, or business development.
• Strong understanding of the OPWDD system and developmental disability services.

Responsibilities

• Develop and maintain relationships with OPWDD Care Managers, CCOs, schools, hospitals, clinics, social workers, and community organizations.
• Educate referral sources on home care, nursing, therapy, and support services.
• Attend networking events, conferences, and community outreach initiatives.
• Serve as a resource for families navigating OPWDD eligibility and service coordination.
• Collaborate with intake, eligibility, and clinical teams to ensure smooth referral conversion.
• Maintain referral tracking and pipeline management within CRM systems.
• Identify new business opportunities and partnerships throughout NYC.
