Job summary

Thresholds is seeking a Community Support Specialist to provide mental health and addiction treatment support in clients’ communities. You will assist with care planning, remove barriers to care, and help clients access resources and navigate complex systems.

You’ll drive clients to appointments and daily activities, adapting to each client’s schedule and needs. The role emphasizes harm reduction, trauma-informed care, and ongoing professional development, with opportunities for licensure

Qualifications

• Passion for mental health advocacy and caring attitude.
• Ability to communicate clearly and build relationships with clients.
• Strong writing skills for documentation and notes.
• Good time management and organizational abilities.
• Must be at least 21 years old and have a valid driver's license with daily access to a car.
• Willingness to transport clients as needed.

Responsibilities

• Provide mental health and substance use treatment support in the client’s environment.
• Assist with care planning and case management to connect clients with resources.
• Help clients navigate benefits offices, doctors' appointments, and daily tasks.
• Support clients in strengthening daily living skills and independence.
