Job summary

Qualifications

• Must pass California DOJ background check and Live Scan.
• Minimum HS Diploma or AA degree in mental health or related field.
• Bachelor’s degree in Social Work, Psychology or related field preferred.
• Four years of experience with HS Diploma in mental health/social services.
• Two years of experience with an AA degree; one year with a BA degree in the field.

Responsibilities

• Provide targeted case management and mental health rehabilitation services in the field.
• Collaborate with clinicians, psychiatrists and inter/intra agency staff.
• Conduct needs assessments and help develop individualized treatment plans.
• Engage clients in goal setting and empowerment throughout services.
• Maintain documentation and ensure service delivery meets standards.
