Job summary

VitalCaring Group is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) for field-based home health care. You will provide direct patient care under the supervision of a Registered Nurse, support care plans, and ensure safe, effective treatment in patients' homes.

You will deliver skilled nursing services, monitor patient conditions, and reinforce education to support successful outcomes while collaborating with the care team and interdisciplinary professionals.

Qualifications

• Active LPN/LVN license in the practicing state.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum 1 year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).

Responsibilities

• Provide direct patient care under supervision of an RN.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
• Support coordinated care by maintaining consistent communication across the care team.
