Job summary

VitalCaring Group is seeking a Licensed Practical Nurse (LPN/LVN) for field-based home health roles. You will provide direct patient care under RN supervision, helping implement care plans, monitor conditions, and deliver treatments in patients' homes.

Ideal candidates have an active LPN/LVN license, CPR certification, and at least 1 year of nursing experience (home health, Med-Surg, or acute care a plus). A reliable vehicle is required for travel.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
