Job summary

VitalCaring is seeking an LPN/LVN to provide hands-on home health care, delivering treatments and patient education under RN supervision. You will support care plans, monitor conditions, and ensure safe, effective care in patients’ homes.

This field-based role emphasizes compassionate, patient-centered visits within a defined territory, with opportunities for professional growth and flexible scheduling.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one (1) year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus).

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self‑care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
• Communicate effectively with physicians, nurses, and interdisciplinary team members.
• Participate in case conferences, team meetings, and ongoing training.
• Support coordinated care by maintaining consistent communication across the care team.
