Job summary

Merakey is seeking an Intensive Case Manager for Adults to join the Carlisle, PA program. The role involves traveling to client homes, delivering intensive case management, and coordinating service plans for adults with serious mental health needs.

Responsibilities include client assessments, linking to resources, family education, and ongoing documentation; on-call duties are included. A valid driver's license is required along with relevant experience or RN licensure.

Qualifications

• Bachelor's degree in sociology, social work, psychology, or related field.
• Or high school diploma with 12 college credits and 2 years in human services with direct client contact.
• RN license also qualifies.
• Valid driver's license and reliable transportation.
• Experience with adults in human services preferred.

Responsibilities

• Travel to meet clients in homes and communities across Cumberland and Perry Counties.
• Deliver Intensive Case Management services to adults with serious mental health needs.
• Conduct assessments, develop service plans, and provide ongoing support via linking, monitoring, advocacy and skill-building.
• Assist clients in accessing psychiatric care, community resources and social supports.
• Educate and support families and significant others.
• Maintain accurate documentation in compliance with regulations.
• Participate in clinical supervision, staff meetings and quality initiatives.
• Be on-call for caseload.
