Job summary

Merakey seeks an Intensive Case Manager for Adults to serve clients in Carlisle, PA. The role involves field visits across Cumberland and Perry Counties, delivering ICM to adults with serious mental health needs and coordinating service plans.

The position requires a relevant degree or equivalent experience, a valid driver’s license, and a commitment to documentation, supervision, and quality improvement. Mileage is compensated.

Qualifications

• Bachelor’s degree in sociology, social work, psychology, or related field; OR high school diploma/GED with 12 college credits and two years’ human services experience, plus direct client contact.
• Valid driver’s license and reliable transportation required.
• Experience working with adults in public or private human services is preferred.

Responsibilities

• Travel throughout Cumberland and Perry Counties to meet clients in their homes and communities.
• Deliver Intensive Case Management (ICM) services to adults with serious mental health needs.
• Conduct assessments, develop service plans, and provide ongoing support through linking, monitoring, advocacy, and skill-building.
• Support clients in accessing psychiatric care, community resources, and social supports.
• Provide education and support to families and significant others.
• Maintain accurate and timely documentation in compliance with Merakey Behavioral Health, County, and State regulations.
• Participate in clinical supervision, staff meetings, and quality improvement initiatives.
• On-call for caseload.
