Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support Medi-Cal members through housing transitions, tenancy services, and deposits. You will travel locally within Mendocino County and surrounding areas, partner with landlords and providers, and document care plans and outcomes in real time.

The role emphasizes independence, strong time management, and compassionate engagement with high‑needs populations while building referral networks and ensuring housing

Qualifications

• Strong understanding of housing system and local housing resources.
• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Proficient with case management systems and EHRs.
• Excellent communication, empathy, and cultural competence.
• Strong organizational and time-management skills.
• Must be proficient with technology and documentation tools.
• Hybrid, field-based work in Mendocino County.

Responsibilities

• Support housing stability through navigation and deposits.
• Coordinate Housing Tenancy & Sustaining Services for long-term housing.
• Assist with Housing Deposits, documenting required paperwork.
• Collaborate with housing authorities, landlords, shelters, and providers.
• Monitor housing stability and address barriers to housing or health outcomes.
• Conduct member assessments and develop care plans.
• Coordinate appointments, services, and long-term housing support.
• Engage with the community and expand outreach/referral networks.
• Document in real-time following state guidelines.
• Travel locally within Mendocino County for field work.
