Job summary

Pacific Health Group seeks a Field-Based Housing Stability Specialist to support CalAIM Community Supports housing programs. You will conduct outreach, intake assessments, and care management for high‑needs members, creating durable housing plans in collaboration with housing authorities and providers.

Expect extensive travel within Monterey County and a hybrid work setup. The role emphasizes independent field work, strong organizational skills, and real-time documentation while coordinating

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs preferred.
• Proficient in case management systems and real-time documentation.

Responsibilities

• Provide Housing Stability Services to help members transition into and maintain stable housing.
• Coordinate Housing Tenant & Sustaining Services with ongoing case management and landlord engagement.
• Assist with Housing Deposits by coordinating documentation and deposit assistance.
• Conduct in-person visits, assessments, and follow-ups in field-based environments.
• Collaborate with housing authorities, shelters, landlords, and community partners to improve access and placement.
• Maintain timely, accurate documentation compliant with state guidelines.
