Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered hospice care in home-based settings, serving Garfield, Major, Alfalfa, and Kingfisher Counties. You will manage symptoms, coordinate care with an interdisciplinary team, and provide education and emotional support to patients and families with compassion.

Ideal candidates hold an active RN license, have at least 1 year of clinical nursing experience, and can work autonomously while delivering high-quality

Qualifications

• Active RN license (or compact eligibility).
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver's license and reliable transportation.
• Experience with Homecare Homebase (HCHB) is a plus.

Responsibilities

• Lead Patient-Centered Hospice Care: Conduct comprehensive patient and family assessments.
• Lead Patient-Centered Hospice Care: Develop and manage individualized, comfort-focused care plans.
• Lead Patient-Centered Hospice Care: Deliver skilled nursing care with a focus on pain and symptom management.
• Serve as the Care Coordinator: Act as case manager, coordinating care across the interdisciplinary team.
• Serve as the Care Coordinator: Collaborate with physicians, social workers, chaplains, and aides.
• Serve as the Care Coordinator: Monitor patient condition and adjust care plans to maintain comfort.
• Support Patients & Families: Provide education, emotional support, and guidance throughout the journey.
• Support Patients & Families: Help families navigate complex decisions with empathy and clarity.
• Support Patients & Families: Build meaningful, trusting relationships in deeply personal moments.
• Drive Quality & Compliance: Maintain accurate, timely documentation.
• Drive Quality & Compliance: Participate in IDT meetings and care planning.
• Drive Quality & Compliance: Support quality and compliance standards
