Job summary

VitalCaring Group is seeking a Hospice Registered Nurse (RN) to deliver compassionate, one-on-one end-of-life care in patients’ homes and facility settings. You will lead the plan of care, manage symptoms, and advocate for patients while coordinating with physicians, social workers, chaplains, and aides.

You will provide education and emotional support to families, ensuring comfort, dignity, and alignment with patient goals.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Lead Patient-Centered Hospice Care: conduct assessments, develop and manage care plans, deliver skilled nursing with pain/symptom focus.
• Serve as the Care Coordinator: act as case manager and coordinate care with the interdisciplinary team.
• Support Patients & Families: provide education and emotional support, guide decisions with empathy and clarity.
• Drive Quality & Compliance: maintain documentation, participate in care planning, uphold standards.
