Job summary

VitalCaring is seeking a Hospice Registered Nurse (RN) to lead patient-centered end-of-life care in homes, facilities, and community settings across Lee, Itawamba, Monroe, and Chickasaw counties in Mississippi.

You will manage the plan of care, symptom control, and family education, delivering compassionate care with clinical excellence and strong autonomy supported by an interdisciplinary team.

Qualifications

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Responsibilities

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management
• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort
• Provide education, emotional support, and guidance throughout the journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments
• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards
