Job summary

Superior Hospice is seeking a Hospice Admissions Registered Nurse (RN) to guide families through the first step of hospice. The role travels to patients’ homes and facilities to assess eligibility, initiate hospice services, and support families with clear explanations of options and Medicare benefits.

Qualifications

• Active Minnesota RN license.
• Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine or similar.
• Outstanding communication, assessment and relationship-building skills.
• Confidence leading sensitive conversations with patients and families.
• Ability to work independently in a field-based role.
• Valid driver’s license and reliable transportation.

Responsibilities

• Travel to patients’ homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
• Perform comprehensive clinical eligibility assessments and initiate hospice services.
• Lead conversations about goals of care, hospice philosophy, Medicare benefits, and what families can expect.
• Coordinate transitions into hospice with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team.
• Build trust with patients, caregivers, and referral partners from the first interaction.
• Complete accurate, timely EMR documentation while ensuring regulatory compliance.
