Job summary

Superior Hospice is seeking an experienced Hospice Admissions RN to guide families through the first step of their journey. You will travel to patients' homes and facilities, perform clinical eligibility assessments, and initiate hospice services with compassion and clear communication.

As part of the Home Health Care, Inc. family, you’ll work in the Greater Twin Cities metro area with a 21-county service footprint.

Qualifications

• Active Minnesota RN license required.
• Experience in hospice, oncology, ICU, palliative care, home health, or admissions nursing.
• Excellent communication and relationship-building skills.
• Ability to work independently in a field-based role.
• Driver’s license and reliable transportation.

Responsibilities

• Travel to patients' homes, assisted living communities, hospitals, and SNFs to complete hospice admissions.
• Perform comprehensive clinical eligibility assessments and initiate hospice services.
• Lead conversations about goals of care, hospice philosophy, Medicare benefits, and expectations.
• Coordinate transitions with physicians, discharge planners, social workers, intake coordinators, and the interdisciplinary team.
• Build trust with patients, caregivers, and referral partners from the first interaction.
• Complete accurate EMR documentation while complying with regulations.
