Job summary

Superior Hospice in the Greater Twin Cities Metro Area seeks a compassionate Hospice Admissions Nurse (RN) to be the first clinical connection for families. You will assess eligibility, educate on hospice services, and coordinate a smooth transition to care in patients’ homes and facilities.

Collaborate with physicians and the interdisciplinary team, document thoroughly in EMR, and advocate for patients during one of life’s most meaningful moments.

Qualifications

• Active Minnesota RN license and hospice/healthcare background.
• Experience educating patients and families about care options.
• Ability to work independently with a flexible field schedule.

Responsibilities

• Travel to patients' homes, assisted living facilities, hospitals, and SNFs for admissions.
• Conduct clinical eligibility assessments and hospice appropriateness decisions.
• Educate about hospice philosophy, benefits, and what to expect.
• Coordinate seamless transition into hospice services.
• Document in EMR promptly while maintaining regulatory compliance.
