Job summary

Boundless Home Health LLC is seeking a compassionate Full-Time Registered Nurse to serve patients across the Fort Worth area in a field-based home health setting. You will travel to patient homes to deliver skilled nursing care, perform assessments, and help manage individualized plans of care.

The ideal candidate communicates well, works independently, and is dedicated to delivering high-quality care in patients’ homes.

Qualifications

• Current and active Registered Nurse (RN) license in Texas (in good standing).
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Provide skilled nursing care to patients in their homes throughout the Fort Worth area.
• Complete patient assessments and monitor changes in condition.
• Develop, implement, and update individualized plans of care.
• Coordinate care with physicians, therapists, caregivers, and other team members.
• Administer medications and treatments as ordered.
• Provide wound care and other skilled nursing services as needed.
• Educate patients and caregivers regarding medications, treatments, disease management, and safety.
• Document visits and patient progress accurately and within required timeframes.
• Report significant changes in patient condition to the appropriate provider or supervisor.
• Maintain compliance with agency policies, clinical standards, and applicable regulations.
