Job summary

VitalCaring Group is seeking a Registered Nurse (RN) for Home Health in field-based service areas including Steubenville, OH and Wheeling, WV. You will lead the plan of care, coordinate across disciplines, and drive outcomes from admission through discharge.

You will apply advanced assessment, care planning, and patient education to reduce hospitalizations and support independence at home. A strong clinical background and independent judgment are essential.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
