Job summary

VitalCaring Group is seeking a Registered Nurse (RN) – Home Health to provide field-based care across Steubenville, OH and Wheeling, WV. You will lead care plans, assess patients in their homes, and collaborate with interdisciplinary teams to improve outcomes.

You will manage complex cases with clinical autonomy, coordinate transitions, and educate patients and families to promote independence and reduced hospital readmissions.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one (1) year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).

Responsibilities

• Conduct in-home assessments and evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including med management and treatments.
• Coordinate across therapists, aides, and physicians as case manager.
• Monitor progress and adjust care based on changing conditions.
• Educate patients and families for disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides and participate in on-call rotation.
