Job summary

VitalCaring Group is seeking a Registered Nurse (RN) for field-based Home Health to lead patient care across an assigned territory. You will assess, plan, implement, and evaluate care, coordinating with physicians and interdisciplinary teams to optimize outcomes.

Qualifications

• Active RN license and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health/Med-Surg/ICU/Acute Care).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments evaluating clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management and treatments.
• Act as case manager coordinating across therapists, aides, and physicians.
• Monitor patient progress and adjust care based on changing conditions.
• Educate patients and families for disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate EMR documentation.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
