Job summary

VitalCaring Group is seeking a Home Health Registered Nurse to act as clinical lead for patients in their homes. You will manage plans of care, coordinate across disciplines, and drive outcomes from admission through discharge.

You will use advanced assessment, care planning, and patient education to reduce hospitalizations and support long-term independence, with strong autonomy and interdisciplinary support.

Qualifications

• Active RN license and CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience; Home Health/Med-Surg/ICU preferred.
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as the case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
