VitalCaring Group is seeking a motivated Occupational Therapist to provide expert home health services in a patient-centered, field-based role. You will evaluate functional status, design evidence-based plans, and collaborate with families and interdisciplinary teams to maximize independence in daily living activities.

The role emphasizes safety, adaptive equipment, and coordinated care, with opportunities to mentor staff and contribute to discharge planning for smooth transitions.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification required.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience; home health experience a plus.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsabilités

• Evaluate patients’ functional status, mobility, strength, coordination, and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals to improve independence.
• Provide therapeutic interventions to restore function and retraining in daily living activities.
• Fabricate splints and train patients on adaptive, prosthetic, and orthotic devices as appropriate.
• Support patients returning to work through functional and work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure patient-centered care.
• Participate in discharge planning to support successful transitions.
