VitalCaring Group in Oklahoma is seeking a Licensed Practical Nurse (LPN/LVN) for field-based home health. You will provide direct patient care under RN supervision, implement care plans, and support recovery and independence in patients' homes.

The role emphasizes one-on-one patient contact, wound care, medication support, education, and collaboration with an interdisciplinary team. A valid Oklahoma LPN/LVN license and CPR are required; drivers license and reliable transportation are needed.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.

Responsibilities

• Provide direct patient care in accordance with the state Nurse Practice Act.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Monitor patient condition and report changes to the RN and care team.
• Educate patients and caregivers on self-care, treatments, and disease management.
