Job summary

VitalCaring Group is seeking a licensed practical/vocational nurse (LPN/LVN) for field-based home health. You will provide direct patient care under RN supervision, implement care plans, and deliver skilled nursing services in patients’ homes.

The role emphasizes ongoing education, coordination with the care team, and nurturing patient relationships while ensuring high-quality, compassionate care in a home setting.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum one year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).
• Strong clinical and observational skills.
• Clear communication and patient education abilities.
• Ability to follow care plans while recognizing and reporting changes in condition.

Responsibilities

• Provide direct patient care under supervision of RN.
• Implement and support plan of care.
• Deliver skilled nursing services including treatments, medication support, and wound care.
• Monitor patient condition and report changes to RN/care team.
• Educate patients and caregivers on self-care and disease management.
• Maintain infection control and proper use of equipment.
• Document patient care accurately in EMR.
• Communicate with physicians, nurses, and interdisciplinary team.
