Job summary

VitalCaring Group is seeking an LPN/LVN to provide direct home health nursing under RN supervision. You will implement care plans, deliver treatments, monitor conditions, educate patients and caregivers, and coordinate with the clinical team.

Ideal candidates have an active LPN/LVN license, CPR certification, and at least 1 year of nursing experience in home health or related settings. A flexible, field-based schedule supports work-life balance.

Qualifications

• Active LPN/LVN license in the state of practice.
• Graduate of an accredited practical/vocational nursing program.
• Current CPR certification.
• Minimum 1 year nursing experience preferred (Home Health, Med-Surg, or Acute Care a plus).

Responsibilities

• Provide direct patient care under the supervision of an RN.
• Implement and support the plan of care developed by the RN.
• Deliver skilled nursing services, including treatments, medication support, and wound care.
• Educate patients and caregivers on self-care, treatments, and disease management.
• Maintain infection control and proper use of equipment and supplies.
• Document patient care accurately and timely within the EMR.
