Job summary

Right at Home SLC is seeking a Case Manager to drive sales and promote the company’s trusted home care options within the community. You will actively visit referral sources and participate in local events to grow market share and provide exceptional service.

The role emphasizes relationship-building, independent work, and on-call availability, with a focus on achieving annual sales goals and strong community presence.

Qualifications

• Associate/Bachelor's in health administration, Social Work, or related field is a plus.
• Able to work independently, demonstrating sound judgment.
• Be available for on-call duty outside normal office hours.

Responsibilities

• Promote sales and create a positive identity through promotional materials and personal visits.
• Increases care hours, develops a profitable referral source pipeline, and gains market share.
• Pre-plans weekly sales activities, communicates with referral sources, and logs all activity in CRM.
• Conveys a strong need to win, stays persistent, and aims to meet annual sales goals.
• Represent Right at Home at local events, health fairs, and networking opportunities.
• Serve as a trusted resource for families exploring home care options.
