Job summary

Care 4 U Community Health Center in Miami seeks a full-time Community Health Outreach Worker (CHOW) to join our outreach team. You will engage individuals living with HIV not in care and those at risk, conducting outreach in Liberty City and surrounding communities to connect them with HIV/STI testing, prevention, PrEP, and linkage to care.

The role is primarily field-based, about 90% outdoors, with occasional evenings and weekends.

Qualifications

• Outgoing, compassionate and comfortable engaging strangers.
• Enjoys working outdoors and walking for extended periods.
• Fluent in English and Spanish preferred.
• Strong communication and interpersonal skills.
• Able to work effectively with diverse populations.

Responsibilities

• Conduct community outreach and recruitment for HIV/STI testing, treatment, and prevention services, including PrEP.
• Provide HIV/STI testing, health education, risk-reduction counseling, and condom distribution.
• Conduct social and health needs assessments.
• Refer and link clients to medical care, prevention services, and community resources.
• Assist individuals in navigating healthcare and support systems.
