The Salvation Army Ferguson Community Empowerment Center is seeking a full-time Pathway to Health Case Manager to recruit, enroll, and apply interventions for clients in Pathway of Hope and Pathway to Health. The role is community-based with frequent local travel in Ferguson, MO.

Responsibilities include pre-screening, intake, goal setting, and regular home visits, with documentation in SIMS & HMIS databases. Travel is approximately 70% community-based; 30% office-based.

Qualifications

• Bachelor’s degree in human services or related field (BSW/MSW preferred).
• 1 year related work experience.
• Outgoing personality with strong communication and cultural sensitivity.

Responsibilities

• Recruit and enroll participants for Pathway of Hope and Pathway to Health.
• Conduct pre-screening and intake using specified assessment tools.
• Set goals with clients and formulate action plans.
• Provide home visits and ensure visit documentation.
