Job summary

Humana Inc. is seeking a Functional Screener in Milwaukee to schedule and conduct in-home, face-to-face assessments for functional eligibility re-determinations, ensuring data accuracy and adherence to guidelines.

You will use DHS screening tools, maintain certification, travel up to 50% across Milwaukee and surrounding areas, and participate in QA activities while upholding privacy and professionalism in all assessments.

Qualifications

• Four-year bachelor’s degree in Health or Human Services with one year of experience OR a Wisconsin licensed RN with one year of experience.
• LTCFS certification required; must complete online screener certification.
• Preferred: LTCFS certification and prior Family Care screener experience.

Responsibilities

• Schedule member appointments for annual or change-based functional eligibility re-determinations.
• Use the DHS screening tool to assess functional abilities, health, and required supports.
• Enter assessment data into the online system and determine level of care.
• Submit documentation promptly and accurately.
• Attend trainings and QA activities to ensure screening competence.
