Job summary

Chelan County seeks an Outreach Case Manager to respond to behavioral health-related 911 calls, provide crisis intervention, and link individuals to appropriate resources in Chelan and Douglas Counties.

The role emphasizes face-to-face interventions, transitional case management, assessments, and coordination with law enforcement to reduce unnecessary incarceration and hospitalizations. Strong collaboration with community partners is essential.

Qualifications

• Bachelor's Degree in Social Behavioral Sciences or a minimum of two years of field experience in intensive case management and/or behavioral health services.
• Minimum of two years experience in the provision of behavioral health services preferred.

Responsibilities

• Works collaboratively with law enforcement on calls involving individuals in mental health/substance use crises.
• Acts as the primary contact for law enforcement and participates in regular ride-alongs with partner departments.
• Performs risk assessments, crisis interventions, and uses de-escalation techniques to stabilize individuals.
• Provides outreach services to divert individuals from incarceration or hospitalization into least restrictive settings.
• Coordinates with crisis services, treatment facilities, EMS, and hospital staff to arrange evaluation and services.
