Job summary

TAG MedStaffing is seeking a Community Registered Nurse (RN) to provide home-based, community nursing and care coordination in Brooklyn, NY. This temp-to-perm, field-based role requires delivering patient-centered care, discharge planning, and clinical support for individuals with IDD and behavioral health needs.

Responsibilities include assessments, medication management, and collaboration with interdisciplinary teams to ensure safe transitions and continuity of care.

Qualifications

• Graduate of an accredited nursing program.
• Active NY RN license.
• Minimum 2 years in community health, behavioral health, care coordination, crisis intervention, or IDD.
• Familiarity with OPWDD, OMH, DOH, and community-based nursing practices.
• Proficient with EMR, clinical documentation, nursing assessments, medication management, and care coordination.

Responsibilities

• Conduct nursing assessments for admissions, complete home visits, and evaluate transitions from hospital to community-based programs.
• Provide skilled nursing services including medication management, chronic disease monitoring, wound care, and health assessments.
• Coordinate hospital discharges, collaborate with healthcare providers, and facilitate continuity of medical, behavioral health, and community-based services.
• Review medication administration records, monitor therapeutic outcomes, oversee medication storage, and ensure regulatory compliance.
• Collaborate with physicians, case managers, behavioral health professionals, families, and direct support staff to develop and implement individualized care plans.
