Job summary

Optum Home & Community Care in New York is seeking an experienced registered nurse (RN) for a field-based role in senior care facilities. Monday through Friday schedule with no weekends, holidays, or on-call requirements.

You will collaborate with an interdisciplinary team to deliver comprehensive clinical care to high-risk members in a facility setting. The position emphasizes care coordination, documentation accuracy, and support for providers and APCs.

Qualifications

• Active unrestricted RN license in New York.
• 1+ years of clinical experience.
• Driver's license and reliable transportation to travel between facilities.
• Ability to lift 30 pounds and perform physical assessments.

Responsibilities

• Conduct accurate and complete patient assessments.
• Collaborate with interdisciplinary care team.
• Assist with care coordination and Medicare benefits processes.
• Document and code accurately, including medication reconciliation and nursing rounds.
• Support providers and APCs in clinical operations and care plans.
• Participate in facility-based meetings and family conferences.
