Job summary

BJC Behavioral Health, a community health entity, seeks a Community Outreach Case Worker to connect clients with housing and food resources and coordinate support in the community. You will travel regionally to meet clients in homes or community settings and document progress in care plans.

Qualifications

• Bachelor's Degree in Human Services or related field.
• No prior experience required.
• Valid driver’s license and access to a personal vehicle.

Responsibilities

• Provides community support services to complex clients to aid recovery and engagement.
• Completes timely documentation detailing service, intervention, and client response.
• Contacts individuals/referral sources after missed appointments to re-engage and support recovery.
• Provides mentorship and job shadowing to community support colleagues.
